Ascension Holistic Care offers Structured Family Caregiving services to clients that are eligible for the "Indiana PathWays" program or direct services from "Family Caregivers".

Family Caregivers

Structured Family Caregiving services provide family members with an opportunity to become caregivers to clients with the Aged and Disabled / Traumatic Brain Injury Waiver. Family members can provide at-home care for loved ones and receive a daily stipend. Our team guides families through the eligibility process, offering home visits and ongoing consultations to support family caregivers.

Indiana Pathways for Aging

Aging at Home with the Indiana Pathways for Aging Program

The Indiana PathWays for Aging program, launched in 2024, empowers Hoosiers aged 60+ to age at home. Through quarterly home visits, annual assessments, and monthly phone check-ins, clients receive comprehensive support that enables them to remain close to family and friends, with access to services such as meal preparation, home-health visits, transportation, and adult day centers. Our healthcare advisers are available to help clients explore eligibility and benefits.
